We feel our responsibilities go beyond our technical expertise. We place a strong emphasis on the patient-company relationship. Our overarching philosophy is that patients need to be treated as people, not numbers and that philosophy permeates all aspects of our company. Patient satisfaction is always our goal, and the history of the success of our company testifies to our dedication toward providing it. We closely listen to each and every one of our patients, tailoring our solutions to your specific needs. We believe it is our job to address your questions and concerns. Our office staff is attentive, informative and understanding. Patient satisfaction is the hallmark of our practice. We continually dedicate ourselves to providing the highest quality service. After each encounter with KOZIE, we want you to feel you’ve been treated with courtesy, warmth, and respect.

 

That’s why we:

  • Provide a quick response to your inquiries

  • Try to minimize wait times for your shipments

  • Maintain a staffed call-line 24/7 for your sleep test questions

  • Provide courtesy reminders when your supply replacement is due

  • Assign a dedicated patient care representative and provide access to them via call and text

We ask you to consider carefully reading our policies below, to determine if we are the best fit for your needs. While we would love nothing more than to serve everyone, we understand, and accept, that our established company policies may not always be compatible with your beliefs. As it is our intent to operate our company with good faith and transparency, we choose to be upfront and transparent with our policies, to enable you to execute your right to choose a company to service your sleep apnea testing and treatment needs.

CHARGEBACK PREVENTION POLICY 

KOZIE WILL NOT TOLERATE THEFT IN ANY CAPACITY. CLIENT understands and agrees that in the event CLIENT initiates a chargeback and/or merchant dispute with their issuing bank for the service/product they have received and in fact owe payment for, KOZIE will make every effort to recover the amount of the chargeback plus KOZIE’S $200 USD investigative fee, in addition to any chargeback fees levied by a payment service provider, including but not limited to having the amount recovered by a collection agency.  

KOZIE will, WITHOUT EXCEPTION, charge a “200 USD Investigative fee” if a chargeback is placed with the CLIENT’s credit card company (either intentionally or unintentionally) for any deposit made to the CLIENT Account. This fee will be used to cover all investigative expenses required. 

ALL SALES ARE FINAL

All sales are final. KOZIE does not offer refunds, returns, or exchanges for any reason.   

 
Exception to the ALL SALES ARE FINAL Policy: Masks, tubing, cushions, and chin straps may be returned if UNOPENED and UNDAMAGED. NO OTHER ITEM(S) MAY BE RETURNED, REFUNDED, OR EXCHANGED. Return for eligible item(s) must be initiated by contacting KOZIE via email (patientcare@koziehomesleeptesting.com) within 5-days of delivery. ITEMS PAST THIS MARK ARE NOT ELIGIBLE FOR RETURN. Eligible item(s) must be registered as “received”, or equivalent, in the carrier’s system within 10 days of original delivery to CLIENT. CLIENT is responsible for all shipping costs. Refund will be issued to CLIENT’s original payment method. Refund will be issued within 7 business days of receipt of items, if applicable. If, at KOZIE’S discretion, the item is OPENED or DAMAGED, a refund will not be issued. CLIENT is responsible for the costs of shipping the item back to CLIENT. If CLIENT does not submit payment for shipping the item back within 10-days of being notified the items will be shipped back, the item(s) will be discarded, and no refund, return, or exchange will be offered.  

ORDER CANCELLATION POLICY 

Restocking fee of 20% will be charged for any cancellation. IF order has shipped, no order cancellation option is offered.  

POST DELIVERY THEFT POLICY 

It is the buyer's responsibility to provide a secure location for receiving packages. If your receiving location is not secure please contact Kozie immediately upon placing your order to have your order securely held at a UPS or FedEx location. Contact Kozie via email at patientcare@koziehomesleepapneatesting.com. If your package is stolen, after being delivered by the carrier, we recommend filing a police report for theft. KOZIE is not responsible for packages being stolen after being marked as delivered by the carrier. REFUNDS WILL NOT BE ISSUED. COMPLIMENTARY SUPPLIES / EQUIPMENT / SERVICES / STORE CREDIT WILL NOT BE SUPPLIED.  

SHIPPING POLICY

In stock items are guaranteed to ship within 7 business days of order and payment receipt. Out of stock items do not have a guaranteed ship time. Out of stock items may take 6-8 weeks, or more, to ship. You may cancel your out of stock order at any time, no restocking fee will apply.  

 

KOZIE is committed to customer satisfaction and guarantees your item(s) is in the carrier’s possession within 7 business days of order and payment receipt. HOWEVER, KOZIE WILL NOT BE RESPONSIBLE FOR DELAYS CAUSED BY SHIPPING CARRIERS. By ensuring the carrier has your item(s) within the guaranteed ship time, we have fulfilled our duties. REFUNDS, RETURNS, OR EXCHANGES WILL NOT BE ISSUED FOR DELAYS CAUSED BY 3RD PARTIES.
